We develop therapeutic settings for recovery, encourage social activity, promote physical enrichment and provide psychosocial health monitoring.
- Comprehensive wrap around services are an important component of community support systems. PAMDC program and services assist caregivers and family members respite from the demanding responsibilities of care-giving. We coordinate professional and compassionate services for diverse adult populations. Services specifically designed to provide social, behavioral and medical stimulation in a supervised environment.
Our mission is to collaborate with health professionals and invite an ecological perspective directed towards environmental resources to reduce potential stressors.
Our goal is to accommodate persons with chronic medical health concerns and/or severe and persistent mental illness. We incorporate an individual-centered strategy that aims at developing the consumer's abilities through enhancing care and interacting with their environment.
PROGRAMs
PAMDC service integration concentrates on coordination of specialized services to incorporate entry points for consumers to have easy access to community resources, agency support, health professionals and other services.
Medical Day Services
We create a platform for respite from chronic disease. Participants thrive in a dedicate milieu for revitalization. Services include medication management, interactive activity, and safe, secure environment for participants requiring supervised daily care.
Psychiatric Rehabilitation
Individuals with severe and persistent mental illness are guide Resources management enrich quality of life and to prevent re-hospitalization and crises. maximize community integration for consumers to enable them to be successful and satisfied in the environment of their choice.
Vocational Services
Consumers are assisted with skills building, mock interviews and resume development. Individuals benefit from a job coach and a committed team to help consumer find gainful employment all in a safe structured environment.
Case Management
Participants work one on one with personal case managers to help with entitlements like Food Stamps, MTA mobility, Medical Assistance/Medicaid, Temporary Cash Assistance, Emergency Energy Assistance, Disability, Housing, Etc.
Network Providers
PAMDC aims to streamline care to provided an umbrella network for participants, clients, patients and consumers. Members benefit from easy access to health care solutions, faster response times and comprehensive service plans. There is no necessity to exclusively use our network, members are able to customize care and utilize only the services they need.
Services
Our services provide a participant centered framework to eliminate barriers in the community based settings. Services are developed to simplify scheduling for individuals who are transitioning into outpatient care while helping them receive comprehensive care.
